Although they are still mourning the death of their son, Tommy and Janet are moving forward with their separation. Tommy is also still trying to maintain his sobriety while now bogged down with new responsibilities, which include caring for his father. Sean Garrity has struck up a secret relationship with Tommy's sister Maggie, and unfortunately for him, he's really starting to fall for her. Sheila finds condoms in her son Damian's room, and asks Tommy to have a conversation with him about it. In confronting Damian, Tommy learns that the teenager is carrying on an affair with one of his teachers, Mrs. Turbody. And when the city's no-smoking ban threatens the firehouse, the crew tries to make things interesting by starting a big wager on whether they can quit smoking.

To make the next hockey game with the NYPD a wild affair, Tommy mends fences with his ex-co-captain, Ryan, who offers to enlist a fireman who is on leave for beating up three cops. As Lou's affair with Sondra heats up, and Mike comes to grips with Theresa's eating disorder, Franco counsels Sean on how to dump Carol, a woman he says is a bad kisser. And after another day at the track with Uncle Teddy and Arlo leaves him with plenty of cash for Janet and Sheila, Tommy is startled when Laura questions his motives and his sanity. Then, after she's the only one who can stop an angry man from attacking the firefighters removing his morbidly obese mother's corpse from her apartment, Laura gains some credibility with the crew.

In an effort to revive Tommy's love life (and morale), the crew plans a night out to the hot-spot Lotus. At the club, Franco gets hit on by an older woman named Alicia. While he's put-off by their difference in age, he's intrigued by her confidence. Financial problems are affecting a few guys in the house. Lou is trying to keeps his financial problems under wraps, but with the Chief asking him to borrow money, its growing increasing difficult for Lou to hide the fact that he was schemed out of thousands of dollars by a Hooker. The Chief is trying to cover the costs on the nursing home his wife is in. Meanwhile, Franco is considering taking the lieutenants exam, and The Probie is considering a transfer to another house somewhere he wont be hassled as the most junior member of the house.

Tommy, Johnny, and Mickey's investigation into Father Murphey's private life leads to tragedy. Laura starts to believe that one of her crew members is secretly harboring romantic feelings for her. Tommy unwittingly becomes involved in a domestic dispute between Sheila and her new girlfriend.

Tommy Gavin is a firefighter and father who's struggling to cope with the daily turmoil of his life. After his wife Janet leaves him, Tommy moves into a house across the street from her to stay close to their three children. However, the proximity of their relationship is difficult. Tommy isn't dealing very well with his estranged wife dating other men and using his kids as spies, paying them to divulge information about Janet's activities. Lifewise, the stress of his job is starting to get to him. Tommy is haunted by the memories of September 11 and even hallucinates the ghosts of the family and friends he lost. His subconscious manifests itself in the images of these former firefighters, predominately his cousin Jimmy who he often confides in. Tommy works hard to conceal his feelings from his firehouse buddies, even though many of them are still suffering as well.
