The overriding issue is the possible closing of St. Eligius due to too much charitable work--in other words, the bottom line. Helen's daughter returns from Switzerland and has gained a huge amount of weight through ingesting chocolate. A couple can't take a chance on their baby being born because of a small issue on the ultrasound. Elliot's father, the veterinarian, shows up with a collie and puts it on a chemo drug. And an incredibly amoral young man causes havoc.
