Stacy's cover officer clearly had neither a gun on his side nor a radio in his rear pocket when he runs into the parking garage. After checking her car, he suddenly has a portable radio, and after diving out of the way of the suspect's car he pulls a gun from his right side and retrieves the radio from his rear pocket.
After the initial homicide of the police officer, Hooker makes procedural errors. First, he was in a much more powerful car than the suspect and given the slight lead the suspect had should have been much closer to the suspect's car. Next, the proper procedure for a suspect that has fled into a structure is to establish a perimeter so the suspect could not flee the building. Once that was done, then the suspect's car could be checked for ownership and evidence and a person search could be made within the building.
In the final shootout, the suspect without reloading fires 12 rounds from a small caliber semiautomatic pistol that at most could hold 7 rounds.
At the funeral, TJ Hooker (William Shatner) is the only officer not wearing a black band on his badge even though he leads the Honor Guard detail. Following the funeral, all the other officers have removed the black bands from their badges even though it is traditional to honor the fallen officer for thirty days.
After the second homicide, the suspect flees in a blue Chevy. Neither the cover officer nor Stacy gave a description of the car to Hooker or Romano, and neither officer had seen the car, yet as they begin to pursue the car Romano radios they are in pursuit of a blue Chevy.
