The restraining order shown near the beginning is actually not legally binding nor enforceable. The form DV-130 is accurately depicted and is used in Ohio and many other states. However, since the court stamp is missing from the second box at the top there is no judge's seal or signing date shown. The date of the hearing is again missing from section 4. The problem arises that all restraining orders (in states using Form DV-130) expire automatically 3 years from the date of the hearing. Without the dated court stamp, or the date indicated in section 4, there is no way to determine when the order begins nor ends.
